Mission Forms

Below you will find forms for a variety of purposes.  While forms are not the most enjoyable aspect of preparing to serve, we are unapologetic about our responsibility to ensure that people are safe and financial disbursements are transparent and aligned with the purposes of our church.  Feel free to download these PDF files and complete them as necessary for your purposes.  Explanations are given with each form below.
 
  • This form is required to be completed by any individual or group requesting funding for their mission project.  The Mission's Team meets monthly to review requests.  Once your request is received, you and/or your organization will be invited to come to our next Mission's Team Meeting to present your project or ministry.  Missions and Service opportunities at Anona are driven by people passionate about helping others.  At the same time, we recognize that there are monetary issues involved in providing care for those in need.  We are committed to having our mission and service funds go toward connecting people in our church with people in need out in the world.  Our is, in other words, hearts and hands approach to serving. 
  • This form is required for anyone desiring to volunteer in Pinellas County Schools.  As we begin to engage more deeply with both Ridgecrest Elementary and Anona Elementary schools, this form will allow people from the church to serve in a variety of functions including: readers, the Math Monday program, All-Pro Dads, assemblies and more.
